Question : changing AD login names

Walking into a new scenario where everyones AD login name is different standard, one could be just first name, the other first initial lastname and some first.last.  I want to standardize all this.  I assume once i change it, the profiles on their machines will have to be redone.  Should i just create new accounts for each person, or just change the existing profile in AD?  it is a 2003 DC

No, they won't.

Profiles are linked to the account SID not the username. The names in AD won't match the profile names on computers, but those profiles will remain active.

For instance, if you change John Doe with username John to JDoe the user profile would remain as "C:\Users\John" rather than being changed.

You may encounter complications if you use roaming profiles based on %UserName%, but local profiles will be fine.
