I am preparing a proposal for:

1) Deployment MOSS 2007 (production environment).
2) Deployment MOSS 2007 (test environment).
3) Development of customized pages using SharePoint Designer 2007.
4) Creation of Workflow solution using Visual Studio 2008 (C#).
5) Technical Documentation.

My questions:

What is the average price per/hour (U.S. $) of the above? (separately)

Exist any template of Technical Documentation for sharepoint?

Obs:
Costumer is a Large company.
Forecast of 250 hours of C # development.
Approximately 25 pages customized.
I Have a Team of 2 developers, 1 analyst web infrastructure, and 1 business analyst.
