Question : Looking for a PDF application that allows me to create forms

Hi,

I would like to create a pdf form on our web page, that will load the details from my database onto a PDF form.  This form should allow the user to correct any erroneous data, and also enter in missing data.  Any ideas on what software I can use to make this possible?
